How Do I Login?

You must first register in order to use the patient portal.

Once registered as a User on the Patient Portal, they can access the Patient Portal at any time to access your Clinical Summaries and educational resources. You must register their email address with our office. If your email address has changed or is not on file with us please

New Patients

Go to “Patient Login
This can also be found in the top right corner of our website.

Enter email address that is on file with our office
If you have not registered your e-mail with our office, please contact us.

You will be prompted to create a Username and Password.
Be sure to keep this information in a secure location.

Setup a Security Question. Make sure you type the answer.
The security question is used to login and reset your password.

Complete setup by logging in with your email address and password.
You’re set!  You are now an existing user.  Questions? Contact Us.

Existing Patients

Go to “Patient Login 
This can also be found in the top right corner of our website.

Enter your username and click the ‘LOG IN’ button.
If you’re having trouble try entering your e-mail or contact us.

You will be prompted for your Password and Security Question.
Click the ‘Submit’ button, once you’ve entered your password and security question.

You are now logged into the Secure HealthCare Portal.
If you’re having trouble or missing a record, please contact us.

Be sure to LOG OUT!
Make sure to always log out of your portal.

Having trouble logging in?

If you cannot remember your Password or Secret Question, you can request a Password Reset after entering in your User Name.
